The following copies are to be submitted for the
registration for Class 1.
1. Date of birth certificate (issued by Municipal office/
Gram
Panchayat/Military Hospital) is to be enclosed with registration form for

the name of the child.
3. Proof of residence & Proof of below Poverty Line (in
case of claiming admn. Under BPL)
4. Number of transfers during the last 7 years should be
mentioned in the columns

registration form clearly (Xerox copies of order may be enclosed).
5. For classes II and above the TC should be countersigned
by the DEO/Area
Education Officer
of the District concerned after getting the provisional
admission.
